We receive referrals from a variety of sources including parents, teachers, daycare workers,
and physicians. In order to bill insurance, we ask that your child’s physician send a referral
for evaluation and treatment via fax to 615-898-7490. After we receive the referral, our staff
will contact you to discuss your concerns and obtain insurance information. As a courtesy,
Bright Beginnings will call and verify insurance benefits. This verification is not a guarantee of
coverage. Once insurance verification/pre-authorization has been completed, a member of the
Bright Beginnings’ staff will contact you to schedule your first appointment. To make your first
visit run as smoothly as possible, we recommend completing the case history form prior to your
evaluation. You can return the completed form to Bright Beginnings via fax (615-898-7490),
email (firstname.lastname@example.org), or mail (1803 Ward Drive, Suite 202, Murfreesboro, TN 37129).
